Missed Appointments / No Show Fee
- As of January 1st, if you do not notify the office you will not be able to make your scheduled appointment within 24 hours prior to your appointment you will be charged a $25 no-show fee.

Account Balances
- All co-pays, balances, deductible fees, non-insurance payment balances must be paid prior to being seen by the doctor. You will be rescheduled should you not be able to make a payment on your account balance.

Called in Refill Request
- Should you call in a prescription request/refill you will be charged a $10 refill request fee. Your appointments are scheduled for you to discuss concerns, changes, and refills please make sure you cover all ofyour basis before you leave the office! If youbring in your prescriptions for refill at a time you do not have an appointmentyou will be charged the $10 refill fee.
